BLUE CROSS/BLUE SHIELD HMO BLUE AND BLUE CHOICE: Patient needs to obtain a referral from their PRIMARY CARE PHYSICIAN.

HARVARD PILGRIM HEALTH CARE: Patient needs to obtain a referral from their PRIMARY CARE PHYSICIAN.

MEDICARE: Patient needs to bring in a written prescription from their physician (PCP or specialist).
